How to get a PhilHealth ID / PhilHealth Identification Number (PIN)

PhilHealth is the government health insurance. What you really get is a 12-digit PhilHealth Identification Number (PIN) that is yours for life. PhilHealth does not run a plastic ID card programme. Your proof of membership is your PIN plus a printed or on-screen Member Data Record (MDR).

The three ways to get it

Online

Yes

This is fully online for getting the PIN. Two hard rules from PhilHealth: one email address can be used only once, and one person can send only one application. Do not submit twice while waiting.

If you already have a PIN, skip the PIN application and go straight to Create Account in the Member Portal.

  1. Download and fill out the PhilHealth Member Registration Form (pmrf) from www.philhealth.gov.ph/downloads/membership/pmrf_012020.pdf
  2. Scan or take a clear photo of the filled form and of your valid ID or PSA birth certificate.
  3. Open memberinquiry.philhealth.gov.ph/member/pinApplication.xhtml
  4. Type a valid email address and answer the captcha, then click Proceed.
  5. Upload your filled pmrf and your supporting document. If these are missing, PhilHealth will throw out the application.
  6. Submit and wait. PhilHealth processes it in three (3) to five (5) working days and emails your PIN.
  7. Once you have your PIN, go back to the Member Portal and click 'Create Account' to make your online account.
  8. Log in to the portal to view, download, and print your Member Data Record (MDR) any time.

Walk-in

Yes

This is usually same-day. If you are employed, you do not have to go yourself.

Give the filled pmrf to your HR department. Under PhilHealth Advisory No. 08-01-2011, the employer files it and PhilHealth sends the record back through HR.

  1. Print and fill out the pmrf, or get a blank copy at the office.
  2. Go to any PhilHealth Local Health Insurance Office (lhio) or PhilHealth branch.
  3. Bring one valid proof of identity, such as a PSA birth certificate or a government ID.
  4. Bring papers for any dependents you want to declare.
  5. Hand in the pmrf and documents at the counter.
  6. Get your PIN and ask them to print your Member Data Record (MDR).

Representative

Yes

A plain signed letter is normally enough. Notarisation is not usually required.

For employed members, the simplest path is not a representative at all: let your HR office file it, which is the official channel.

  1. Write a simple authorisation letter. Put your full name, your PIN if you already have one, the full name of your representative, and the exact task you are allowing (for example, 'to get a copy of my Member Data Record').
  2. Sign and date the letter.
  3. Give your representative the letter, a photocopy of your valid government ID, and their own valid government ID.
  4. Your representative brings all of these to the PhilHealth office.

What to bring

  • A filled-out PhilHealth Member Registration Form (pmrf). You can download it from the PhilHealth website.
  • One valid proof of identity. A PSA birth certificate or any valid government ID works.
  • A valid email address. Online registration needs one, and each email can only be used once.
  • An active mobile number
  • If you are adding dependents: papers that prove the link, such as a marriage certificate or the child's birth certificate
  • For foreign nationals: the pmrf-fn form instead, plus passport and visa or ACR I-Card

Who can apply

Every Filipino can and should be a member. This covers employees, self-employed people, freelancers, online workers, OFWs, and people with no work.

Foreign nationals living or working here can register too, using a separate form.

Cost

Free. Registering and getting your PIN costs nothing.

Printing your MDR at a PhilHealth office is free. You only pay your monthly premium contribution, which is a separate thing from the ID. Beware of any site or fixer charging a fee to 'get your PhilHealth ID'.

How long it takes

Online
PhilHealth processes an online registration in three (3) to five (5) working days, then emails you your PIN. Walk-in at a PhilHealth office: usually same day.
Through your employer
depends on how fast HR files it.

How long it lasts

Your PIN never expires. It is permanent and you use the same number your whole life, even if you change jobs, move, or stop paying for a while.

The MDR has no expiry either, but hospitals and HR usually want a recent print, so print a fresh one when you need it.

Tips

  • Never pay for a PhilHealth ID. Registration, your PIN, and MDR printing are all free.
  • Do not register twice. One person, one PIN, for life. Two PINs will split your contribution record and cause trouble when you claim.
  • Save a PDF of your MDR on your phone. Most hospitals now accept a printed or on-screen MDR, and many can check your record directly through their PhilHealth portal.
  • If you lost your PIN, do not apply for a new one. Ask PhilHealth to look it up using your name and date of birth, or check an old payslip or your HR record.
  • As a freelancer or VA, register as a Direct Contributor (Self-Earning Individual) on the pmrf, not as Employed.

The research could not confirm these against an official 2026 source. Phone the office before you travel.

  • PhilHealth does not publish one national rule for authorisation letters, so branches differ. Some ask for a fresh letter for each visit and each transaction, and some want a notarised Special Power of Attorney for anything about money or claims. Call the branch before sending someone.
